Submit the receipts of medical treatment and medicine, a copy of an account, and alien registration card to the Office of International Affairs

The total cost of medical treatment and medicine except one's share is to be deposited into his/her account.

* If the cost is over 200,000, submit the copied report of the first medical examination

2.preexisting condition
3.insanity or mental disorder of an insurant
4.pregnancy, childbirth, stillbirth, or other related surgical operation or medical treatment of an insurant
